OPERATING ROOM RENOVATION PROJECT MANAGEMENT : A CASE STUDY OF A JOINT COMMISSION INTERNATIONAL (JCI) ACCREDITED PRIVATE HOSPITAL

Abstract

The purpose of the research is to study the problems and effects of construction and renovation in the operating room by analyzing the cause for improving efficiency in construction management in accordance with the requirements of Joint Commission International (JCI) Accredited Private Hospital in the category of Construction Management. The collection of data is done by interviewing 15 persons in construction and renovation work in the private hospital that has been certified by the international standard as case study.

According to the interview, the problems found during the pre-construction phase are management and preparation in terms of design standards as well as working regulations on the hospital and operating room area. Ones found during the construction phase are of security and safety protocols. During the post-construction period, issues are found in the evaluation and briefing personnel of relevant information. The impacts that affects pre-construction phase the most are quality control followed by time management. For the construction phase, the most impacting factors are hygiene control and pollution from construction site such as dust. Following the impacts mentioned are the image of professionalism, safety, security and time, respectively. For post-construction, the most effecting factors are quality control and safety followed by the impact of the service, operation, time and pollution, respectively.

Therefore the researcher proposed guidelines for the development of construction management to improve the operating room in accordance with international hospital standard guidelines as following: implement the physical resource database of the hospital as close to “as-built” drawings as possible; develop the construction workflow to be more consistent; provide standards and specific requirements for construction operations to contractors for reference before contractor selection process; lastly, arrange meetings during the construction or renovation on a daily or weekly basis for thorough planning, problem solving and communicating between departments within the organization.

การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อศึกษาปัญหาและผลกระทบจากงานก่อสร้างปรับปรุงห้องผ่าตัด และนำมาวิเคราะห์ถึงสาเหตุ เพื่อนำไปใช้พัฒนาการบริหารโครงการก่อสร้างปรับปรุงห้องผ่าตัด ให้มีประสิทธิภาพและเป็นไปตามข้อกำหนดตามมาตรฐานสถานพยาบาลสากลในหมวดที่เกี่ยวข้องกับการก่อสร้าง โดยเลือกโรงพยาบาลเอกชนที่ได้ผ่านการรับรองมาตรฐานสถานพยาบาลสากล เป็นกรณีศึกษา และเก็บข้อมูลจากการสัมภาษณ์บุคลากรที่เกี่ยวข้องกับงานก่อสร้างปรับปรุงในโรงพยาบาลจำนวน 15 คน

จากการสัมภาษณ์พบว่า ปัญหาในช่วงก่อนการก่อสร้างจะพบปัญหาด้านการจัดการและตรียมการ ในส่วนของข้อกำหนด มาตรฐานการออกแบบ รายละเอียดข้อบังคับในการทำงานในพื้นที่โรงพยาบาลตลอดจนพื้นที่ห้องผ่าตัดมากที่สุด ส่วนในช่วงระหว่างการก่อสร้าง จะพบปัญหาด้านการควบคุม ในส่วนที่เกี่ยวกับความปลอดภัยและการรักษาความปลอดภัยมากที่สุด และในช่วงหลังการก่อสร้างจะพบปัญหาด้านการประเมินผลและการให้ข้อมูลบุคลากรที่เกี่ยวข้องมากที่สุด ในส่วนของผลกระทบในช่วงก่อนการก่อสร้างพบผลกระทบในเรื่องคุณภาพมากที่สุด รองลงมาจะเป็นเรื่องของเวลา ส่วนในช่วงระหว่างการก่อสร้างผลกระทบที่มากที่สุดคือเรื่องชีวอนามัยและการติดเชื้อจากงานก่อสร้าง เช่น ฝุ่นละออง เป็นต้น ตามมาด้วยผลกระทบเรื่องภาพลักษณ์ การปฏิบัติงาน ความปลอดภัยและการรักษาความปลอดภัย และ เวลา ตามลำดับ และผลกระทบภายหลังการก่อสร้างที่พบมากที่สุดคือ เรื่องของคุณภาพของงานและคุณภาพด้านความปลอดภัย ตามด้วยผลกระทบด้านการให้บริการ การปฏิบัติงาน เวลา และด้านการติดเชื้อตามลำดับ

ผู้วิจัยจึงเสนอแนวทางการพัฒนาการบริหารงานก่อสร้างปรับปรุงห้องผ่าตัด ตามแนวทางมาตรฐานสถานพยาบาลระดับ ดังนี้ ปรับปรุงฐานข้อมูลด้านทรัพยากรกายภาพของโรงพยาบาลให้ตรงกับความเป็นจริงอยู่เสมอโดยเฉพาะแบบ As-built drawing พัฒนามาตรฐานหรือขั้นตอนในการทำงานก่อสร้างปรับปรุงโรงพยาบาลให้เป็นรูปธรรมมากขึ้น จัดให้มีการชี้แจงมาตรฐานและข้อกำหนดเฉพาะในการดำเนินงานก่อสร้างปรับปรุงโรงพยาบาลเพื่อให้ผู้รับเหมาใช้ประกอบการเสนอราคาก่อนทำการคัดเลือกผู้รับเหมา และจัดให้มีการประชุมระหว่างการดำเนินการก่อสร้างปรับปรุงอาคารเป็นประจำทุกวันหรือทุกสัปดาห์ เพื่อช่วยในการวางแผนแก้ปัญหาและการสื่อสารระหว่างแผนกภายในองค์กรให้ทั่วถึงกัน
