Abstract: แม้ว่าจะมีการพัฒนางานด้านวิชาการต่างๆ การเพิ่มงานบริการและบริบาลเภสัชกรรมทั้งผู้ป่วย นอกและผู้ป่วยใน แต่ปัญหาการใช้ยาก็ยังไม่ลดลง จึงเกิดแนวคิดให้เภสัชกรโรงพยาบาลลงพื้นที่เยี่ยมบ้านผู้ป่วย เพื่อค้นหาปัญหาที่อาจเป็นสาเหตุที่แท้จริงที่ส่งผลให้เกิดปัญหาการใช้ยาของผู้ป่วยที่กลับบ้าน งานเยี่ยมบ้านโดย เภสัชกรโรงพยาบาลสมุทรสาคร ที่ดาเนินการอยู่ในปัจจุบันภายใต้หลักการดูแลต่อเนื่องด้านยาของผู้ป่วยจากโรงพยาบาลสู่ บ้าน เป็นระยะเริ่มต้น ระบบการทางานยังไม่ชัดเจน ยังไม่มีหลักเกณฑ์ในการทางานบางขั้นตอน จึงจาเป็นที่จะต้องพัฒนา ระบบงานให้เกิดความชัดเจน เหมาะสม เพื่อเพิ่มประสิทธิภาพการปฏิบัติงานและก่อให้เกิดประโยชน์กับผู้รับบริการอย่าง สูงสุด ทั้งนี้ยังทาให้เกิดระบบบริการสุขภาพที่ต่อเนื่องจากโรงพยาบาลสู่ครอบครัวผู้ป่วยที่ดีและเหมาะสมขึ้นอีกด้วย การศึกษาเชิงคุณภาพในรูปแบบการวิจัยเชิงปฏิบัติการครั้งนี้ มีวัตถุประสงค์ เพื่อศึกษาสถานการณ์และการพัฒนาแนว ทางการเยี่ยมบ้านผู้ป่วยของเภสัชกร โดยใช้หลักการดูแลต่อเนื่องด้านยาของผู้ป่วยจากโรงพยาบาลสู่บ้าน รวมถึงการให้การ บริบาลเภสัชกรรมและการสัมภาษณ์แบบไม่เป็นทางการที่บ้านของผู้ป่วย เก็บข้อมูลตั้งแต่เดือนมิถุนายน ถึงธันวาคม 2558 กลุ่มตัวอย่างเป็นผู้ป่วยที่มีปัญหาด้านการใช้ยารวม 15 ราย ได้แก่ ผู้ป่วยโรคเรื้อรังที่มีความเสี่ยงใช้ยาผิด ผู้ป่วยที่ต้องเข้า รักษาตัวในโรงพยาบาลซ้า ด้วยปัญหาด้านยา หรือผู้ป่วยเฉพาะ เช่น ผู้ป่วยโรคไต โดยผ่านการคัดกรองและส่งต่อจากเภสัช กรหรือหน่วยงานเยี่ยมบ้านอื่นในโรงพยาบาลสมุทรสาคร สถิติที่ใช้ คือ ความถี่ ร้อยละ ค่าเฉลี่ย เภสัชกรได้ทาการเยี่ยมบ้านผู้ป่วย 15 ราย รวม 47 ครั้ง จานวนครั้งของการเยี่ยมบ้านเฉลี่ย 3 ครั้ง/ราย ราย ที่มีการเยี่ยมบ้านมากที่สุด 7 ครั้ง และน้อยที่สุด 1 ครั้ง พบปัญหาการใช้ยา 40 เหตุการณ์ มีผู้ป่วย 7 ราย ที่ระบุว่าการ ได้รับยาของตน ก่อให้เกิดความทุกข์รวม 29 เหตุการณ์ ผู้ป่วย 10 ราย มีปัญหาจากตัวผู้ป่วยเอง ซึ่งส่งผลกระทบต่อการใช้ ยาทั้งหมด 125 เหตุการณ์ เภสัชกรได้จัดการปัญหาที่พบจากการเยี่ยมบ้าน แบ่งเป็น 2 กลุ่ม คือ จัดการปัญหาที่เกี่ยวข้อง กับผู้ป่วยโดยตรงและจัดการปัญหาที่ต้องประสานงานกับหน่วยงานอื่นๆที่เกี่ยวข้อง พบว่าการเยี่ยมบ้านผู้ป่วยทาให้ผู้ป่วยมี แนวโน้มจัดการเรื่องยาและดูแลตนเองได้ดีขึ้น ที่ปัญหาต่างๆ ที่ส่งผลต่อการใช้ยาหรือสุขภาพของผู้ป่วยลดลง ผลการ ประเมินขั้นของพฤติกรรมของผู้ป่วยและการจัดกิจกรรมเพื่อส่งเสริมการปรับเปลี่ยนพฤติกรรมตามขั้นของพฤติกรรมของ ผู้ป่วย พบว่าผู้ป่วยมีแนวโน้มการปรับเปลี่ยนสู่ขั้นตอนที่ดีขึ้น กระบวนการที่ใช้ในการเยี่ยมบ้านในการศึกษานี้สามารถใช้ได้ จริงและควรเพิ่มการพัฒนาทักษะสาคัญที่เภสัชกรจาเป็นต้องใช้ในการเยี่ยมบ้าน พบว่าการสื่อสารข้อมูล Case ผู้ป่วยที่มี ปัญหาให้แก่วิชาชีพอื่นที่เกี่ยวข้องเพื่อช่วยดาเนินการแก้ไขนั้น ต้องใช้การติดต่อประสานงานโดยตรงที่หน่วยงานนั้นๆ ไม่ สามารถใช้การส่งข้อมูล Case ผู้ป่วย ผ่าน Note Photo ในโปรแกรม HOSxP ได้ ดังนั้นควรมีการพัฒนารูปแบบหรือช่อง ทางการสื่อสารระหว่างวิชาชีพที่เกี่ยวข้อง เพื่อให้เกิดระบบการสื่อสารข้อมูลระหว่างกันอย่างต่อเนื่องและเหมาะสมต่อไป Although, there were many developments of pharmaceutical services for outpatients and in-patients, drug use problems were not diminished. Then, the idea of pharmacist home visits was commenced to figure out the possible origin of the problem which stimulated patient’s drug use problems at home. Pharmacist home visits in Samutsakhon Hospital, under operation of Seamless Pharmaceutical Care, was just at the beginning. Working system was not perfectly operated and lacked of some relevant procedures. It needed developing to be accurate and efficiency for increasing the operations to reach the highest benefit for patients. Furthermore, it created the continuing healthcare service system from the hospital to patient’s families. The purposes of this Participatory Action Research were to study the situation and development of pharmacist home visits guideline using the continuing Seamless Pharmaceutical Care. Pharmaceutical care and informal interviews at patient’s home were used too. The data was collected from June to December 2015. The studied samples were 15 patients who had a drug-used problem, such as chronic disease patients who had the risk of drug misuse, the re-admission patients from drug use problems or specified patients such as kidney disease patients. The patients were screened and sent from pharmacists or other healthcare persons in Samutsakhon Hospital. Frequency, percentage and average were used to analyze the data. The pharmacist visited 15 patients’ homes, 47 times. The average visit was 3 times/patient. The most visit was 7 times and the least was 1 time/patient. There were 40 drug use problems and 7 patients informed that their prescribed drug caused 29 sufferings. Ten patients caused their own problems with affected drug use 125 times in total. There were 2 groups of resolutions of the problems found from the visits, directly solving problem with the patients and solving problem by coordinating with other associated departments. The home visits resulted in better drug use management of the patients from the problems which affected in decreasing patients’ drug use and health. Patients showed the progress of changing in their health behavior when evaluated them by TM Stages of Change Model. The guideline of Pharmacist home visit could be used as regard adding for essential skills for pharmacists to be used in home visits. Directly contact to other healthcare departments might be more suitable than Note Photo in HOSxP programme in order to communicate the patient’s information for seeking help.
